This app enables healthcare providers to refer patients to Hospice of Philadelphia, learn about our services, and view pictures of our Inpatient Care Unit. Guidelines on hospice eligibility are delineated in the app. All referrals submitted through this app will be received by Hospice of Philadelphia’s Intake Department for follow-up. All patient information received through the referral form will be managed with HIPPA compliance.
